  Submit this form to officially register your Master Guide Club with the Oklahoma Conference. Required annually for club recognition and participation in conference events.

• Master Guide Member Registration
  Use this form to enroll individual Master Guide participants for the current year. Includes basic personal and contact information for each member.

• Master Guide Under 18 Consent Form
  This form is required for any Master Guide participant under the age of 18. It includes parental/guardian permission for involvement in club activities.

• Master Guide Consent to Treat
  Provides medical treatment authorization in case of emergency during Master Guide activities. Must be completed for every participant and kept on file by the club director.
