Welcome to Annual Club Registration!

Every Adventurer and Pathfinder Club across the Oklahoma Conference is required to complete registration at the start of each ministry year. This process helps us stay connected, ensures compliance with safety and communication standards, and allows your club to access conference-wide support, events, and resources.

Whether you're a brand-new club or returning for another year of ministry, completing the annual registration is essential—and easy.

STEP 1: Club Membership Application

Captures key contact and membership info.

STEP 2: Club Commitment Form

  • Signed by the club member and parent/guardian.
  • Confirms participation expectations and conduct standards.

STEP 3: Health & Permission Record

  • Provides emergency contact, allergy, and medical info.
  • Required for participation in any events or off-site activities.

Monthly Report

To help the conference stay up to date with club activity, directors should also submit monthly reports. These are quick check-ins and help us support your club throughout the year. These are also required to achieve Honor Club.

Important Notes

  • Submit one copy of all completed forms to:
